Community Support Individual - Child & Adolescent

To help our customers reach their goals and enable them to live useful, rewarding lives, we provide the following services: linkage to needed resources, advocacy, monitoring, service planning, coordination, assessment, training and supervision.


Admissions
Criteria:

  • Agree to receive services and
  • Be under 18 years old and
  • Have a severe and persistent mental illness or
  • Have substance abuse
  • Have co-occurring substance abuse and mental illness or
  • Have co-occurring mental illness and mental retardation or
  • Individual may need assistance with developing or maintaining skills to develop, maintain, and/or enhance social supports or other community coping skills and
  • Individual may need assistance with daily living skills including coordination to gain access to necessary rehabilitative services and
  • The individual's level of functioning does not preclude the provision of services in an outpatient level of care.

Exclusions
Criteria:

There is no outlook for improvement with this level of care.

Individuals with the following conditions are excluded: mental retardation, autism, organic mental disorder.
