Community Support Individual - Adult

To help our customers reach their goals and enable them to live useful, rewarding lives, we provide the following services: linkage to needed resources, advocacy, monitoring, service planning, coordination, assessment, training and supervision. |

Admissions
Criteria: |
- Agree to receive services and
- Be over 18 years old and
- Have a severe and persistent mental illness or
- Have substance abuse
- Have co-occurring substance abuse and mental illness or
- Have co-occurring mental illness and mental retardation or
- Individual may need assistance with developing or maintaining skills to develop, maintain, and/or enhance social supports or other community coping skills and
- Individual may need assistance with daily living skills including coordination to gain access to necessary rehabilitative services and
- The individual's level of functioning does not preclude the provision of services in an outpatient level of care.

Exclusions
Criteria: |
There is no outlook for improvement with this level of care.
Individuals with the following conditions are excluded: mental retardation, autism, organic mental disorder.
