Community Integration Services - Adult

We believe our customers should have opportunities to choose and pursue their own goals. It is our responsibility to provide the needed supports and services allowing this to happen. Our objective is to help the people we serve integrate into the community and increase the control they have over their lives and activities. We encourage our customers to develop skills and supports necessary to prevent relapse and re-hospitalization.


Admissions
Criteria:

  • Agree to receive services and
  • Be over 18 years of age and
  • Have primary mental health issues that are either incapacitating, distressing or destabilizing and
  • Have deficits in daily living, social skills, community/family integration
  • A functional level that precludes services in less restrictive level of care

Exclusions
Criteria:

  • Requirement of one-to-one supervision for protection of self or others
  • Individuals with the following conditions are excluded from admission unless there is clearly documented evidence of an acute psychiatric episode overlaying the primary diagnosis: autism, organic mental disorder, primary substance abuse problems
  • Active use of unauthorized drugs or alcohol

Services:

  • Training in adaptive living activities
  • Education on mental illness, medications and relapse prevention
  • Linkages to other HRCSB programs and other social services
  • Pre-vocational and vocational-related activities
 
  • Individual/Family Counseling
  • Medication Monitoring
  • Transportation
  • Psychiatric evaluation
  • Advocacy
  • Service Coordination
